i worked of free in MD clinic for three years .During this time i did P.exam ,History even many time wrote Prescription MD just signed it .
what should i put on CAF

Write observership
I did the same thing like you, My attending doctor even did not re-checked what I prescribed because he trusts me.
But I wrote observership in CAF, this is best way because you have no license to practice in the US.

Never tell them YOU Prescribed. Otherwise write it as observership/work experience/ maybe even extern thats fine!

observership,not externship .externship is in hospital setting
