Petechiae in antecubital fossa? I think the pt is injecting drugs.

So, my answer is C. vancomycin.

I put vancomycin as well but for completely different reasons.
First petechiae in antecubital fossa....IV drugs?? petechiae are not track marks! Besides this is USMLE if he was an IV drug user they would tell us. Petechiae are either due to his thrombocytopenia from the AML or from a staph infection causing the petechiae. Being as he's immunocompromised and was previously hospitalized I'm thinking maybe a MRSA sepsis..??

febrile neutropenia..... ticarcillin and tobramycin .. Petichia here i would say is due to thrombocytopenia and not any specific infection.
vancomycin can be added if there is any mucosistis.
amphotericin can be added after 3-5 days if the fever doesnt come down

not a cz it doesnot cover psudomonas

b bcz the doc for febrile neytropenia is cefipime /ceftazidime or antipsudomonal penicillin plus gramnegative drug

so b is the answer

petechia again are due to thrombocytopenia

I changed my answer. I think it's B too! Need broad-spectrum, and definitely need to cover Pseudomonas.
