D Proceed with the surgery as she has given the consent.In emergency U do what U can to save life which is to transfuse blood,U don't withold based on religious bases. If it's not an emergency then U can get court order for something that can affect the patient or other people around the patient

(C) The physician can use alternative means of replacing lost body fluids but should not give the patient a blood transfusion if the patient that made the decision is legally competent.

C; pte is competent and she can decide about what is the best for her. Jehova's witness doesn't accept blood from other person, but I think they accept auto-transfusion and the Surgery can go with some risk.
