Why A?

I know this is pte has 30% obstruction of carotid artery but he is symptomatic, and that also is an indication for stent. Isn't it?

This pt is not receiving any drug which can prevent thrombus formation. The next step shud be Aspirin which is effective for prevention of arterial thrombi.

next best step would be aspirin. but the best thing would be carotid endarterectomyy for a person who is symptomatic with a proven carotid artery stenosis.

You wouldn't ever do an endarterectomy for a pt with 30% stenosis just because he's having TIA symptoms. With 30% stenosis the symptoms are not coming from the carotid and you are just risking an interop stroke. It is more an indication that there is atherosclerotic disease elsewhere, like the circle of willis etc.

Choose carotid endarterectomy over aspirin if the degree of carotid stenosis is>70%
