I they specifically ask for 4 LORs then I assume its MSPE+4 LORs

MSPE does not count for LOR. That is why its name was changed from dean's letter to MSPE. The MSPE has to follow some criteria, look at ir in aamc.org. If your MSPE is more like and LOR from your dean then you should list it like an LOR and in that case you have space for 3 more. If your MSPE is truly and MSPE then you have space for 4 LOR's because an MSPE is deffinitely not an LOR. Look at the guidelines and you'll see.
