I guess U.S Letter of recommendation as most people do have strong letters from their own country and most programs I have heard donot weigh them a lot.

Why don't you use both? You have plenty of space why choose either one? I would use both.... unless your Indian doctor is very famous as poor as your US LOR looks it might be more valuable....but I would use both

Hey thanks for all the answers...I have two other US LORs..this thrid LOR is very poor in the sense that it doesn't say anything about my skills, he just copied down what I did after medical school from my CV...so I was a little afraid to submit that...wondering if such a so so letter would affect the application...also most of the programs want only three LORs...so if we submit a fourth one, do they check all four, or do they discard the fourth one...?
