Hi guys, I rememeber this question from NBME,please answer: A baby is born at 28th weeks gestation,what drug should be given for surfactant deficiency. Choices(i dont rememeber all of the choices) 1)imdomethacin 2)prednisone there were three more but i forgot,please answer this question.tnx.

Or is it that we don't give any drug because surfactant is already being formed by the neonatal lungs?If so then by which week fetal lungs start making surfactant?

Cortisol Increases the Production of Surfactant and it is started to produced by 28th wk of pregnancy. I did reply to ur post earlier but i think it never got posted.

if the baby is borne then u dont give steroids for it. i dont know about the answer to this qs as it is incomplete but artificial surfactant is available and then only the symptomatic traetment of RDS.

Yes Cortisol is to be used Antenatally but after birth Assited ventilation and Aritficial Surfactant is Rx of choice.

Tnx guys,today i was listening to Goljan audios for respiratory system and yes he said that cortisol produces surfactant.Tnx NNL.

I agree with new_n_lost that after birth we give artificial surfactant injected in the lungs directly. there is one fact that should give dexamethasone IM before 33-33 weeks. after that no benefit.
