Saw a cardio q that described pt with dyspnea, cough, amyloidosis, and among the choices were restrictive cardiomyopathy and dilated cardiomyopathy. I chose restricitve, but correct answer was dilated, and just read in FA that amyloidosis connected with restrictive, but in parentheses, says it can also be dilated mad So what do you think I could have missed in the vignette that pointed to dilated? The dyspnea and cough?

Can u Go thru the Question Again Edie. Amyloidosis can cause DCM but its WAY , WAY down the list. Amyloidosis is very frequently associated with RCM. With DCM they ill give u something else a history of myopcarditis or drug abuse history either cocaine or tricyclics. or a hypothroid state.

I tried looking for it, cannot find it now! I thought is was from USMLE CD, checked, maybe from Kaplan.

The dyspnea and cough might be referring to asthma which is a symptom of left sided heart failure. With heart failure the patient presents with dilated cardiomyopathy not restrictive. Hope that helps.

hey edie from where did u prep ur patho?hv u touched goljan rapid review or BRS PATH?ANS TO UR QS IS THERE IN BOTH BOOKS hmmmmm.brs mentioning in BOLD LATTERS dat CARDIAC AMYLYDOSIS is assosiated wid restrictive cardiomyopathy,mentioning no other condition only cardio amylydosis.in GOLJAN RR he is gving 4-5 conditions associated wid rest cardiomyopathy nd AMYLYDOSIS is one of them.both these books nt nt associating amlydosis wid DILATED cardiomyopathy hmmmm.so cheerup,u did hit the rit ans.

etiology of DILATED one is MOSTLY IDOPATHIC(unknown) other factors are THIAMENE DEFICIENCY (wernick korsakof synd) and ofcoyurse alcohlism again leading to thiamene deficiency.while etiology of REST. one is mainly CARDIC AMYLOIDOSIS (acc 2 BRS) nd acc 2 GOLJAN UNCLE amyloidosis + tropical endomyocardial fibrosis+3 other conditions (no need to mention)

clinical features of DIL;ATED ONE acc 2 GOLJAN UNCLE is systolic ejection murmur only and of REST. ONE is CHF which is also CORELATING wid ur SENARIO i,e DYSPNEA,COUGHsmiling facesmiling facesmiling face
