Your 35 y.o. female Pt of 5 years comes to see you with her husband of 2 years. They are both successful lawyers, and the wife has told you recently that she was up for making partner in her firm and has no desire to have children any time soon. You gave her OCPs 1 year ago, and she is doing well with them. However, she has not told her husband, as they have argued in the past about starting a family. To keep peace, the wife has kept her contraceptives a secret. However, the husband fears that there is something wrong with his "boys" as "we have been trying really hard to have a baby for a year and--nothing". The wife tells you that they are "interested in fertility tests, but wanted your advice first". The husband is visibly upset and concerned about the possiblity that he may be infertile. What do you do? a. Agree to run tests on the husband per the couples wishes, say nothing else about it b. Recommend marital counseling and call it a day c. Ask the husband to leave the room, and ask wife to tell him the truth d. Tell the husband that his wife has been on OCPs and that there is probably nothing wrong with him except having an overly ambitious wife e. Refer the couple to the fertility clinic, but advise the wife to consider telling husband the truth

seriouslly i thought that it doc should tell the man in front of wife but didnt like the overambitious part....thought it was a bit weird

Is that what you would do in real life, Hope? For me, I would probably do B just to remove myself from the wife's web of deceit, and leave it for the shrinks to untangle, LOL.

the only reason ONE wud chose D is THAT I GAVE THOSE OCPs & They BOTH r my pts now wht do i do. Otherwise if the Wife is the Pt only then go for E

Answer is E. PHYSICIAN HAS TO MAINTAIN CONFIDENTIALITY IN ALL CIRCUMSTANCES EXCEPT if 1. Their patient is suspected of child or elder abuse.2.their pt has a significant risk of suicide 3.their pt poses a serious threat to others.

C. Ask the husband to leave the room, and ask wife to tell him the truth why can`t this be the right answer? here we are not either breaking the confidentiality... and secondly we are trying to persuade the wife to come clean her self.. and if we refer them to fertility clinic that means we are first of all refering a case " which is Big No in Usmle "

i wud mark C .. coz here the doc is maintaining the confidentiality of the wife. but since her husband has also visited him..just sending them to a fertility clinic wont be wrong on his part ?? because its him who has given the wife OCPs and he knows that probably there is nothing wrong with the husband !! some of these BS qs are soo confusing !!

The answer is C there is no way you would run further tests on this guy, and put him through the emotional stress. By doing option E, yes your protecting the womans rights, but your intentionally decieving your other patient.
