rise in intrathoracic pressure causes heart pumping difficult and venous return is greatly decreased so it is cardiac failure and not anything else ,further sys. venous pressure is high in this not low

it sounds like atelectasia of the left lung too but that is not one of the choices... either way it is a bad accident uh ! Hope that you post the answer too

The patient definitely had tension pneumothorax. Agree with D.
