Now, when I finished with all the exams I can say it is important not to think about Cs like a joke. It's not so hard, like step1/CK, I mean you don't have to study few months 10-12 hours/day but it is important to practice with someone timing and to think it is REAL. I was lucky because I liked all the patients ( I liked people anyway but I thought I will be very stressed and very nervous) and I think this is something what matters because I could relax and talked with them like I would do with my real patients. The cases weren't hard- usual presentations, many possible differential, all what it supposed to know any psysician. I took it in Philadelphia and the staff was great- very nice and very funny and this was another "relaxation" part! I studied from USMLEWORLD and FA and was enough for me. Good luck to everybody.

such a nice sentence: I like people anyway. I'm sure you will pass CS, because SPs also like you. take care. Just one question: Did you feel that SPs show any emtion regarding satisfaction or unsatisfaction with your performance or they seemed neutral?

No, I didn't. One of the patients answered in just 2-3 words, she didn't want to look at me and I thought it was something wrong with me!! But they didn't show any emotion regarding my performance.They really are good actors!

my experience was a bit different. some 4 patients smiled a lot at me and nodded reassuringly as i left them saying the final ggodbye. on the whole i thought i was able to discern from their faces if i had done well or messed it up. from that i thought there was one case that i really mesed up, and two where i wasn't getting to the diagnosis, or at least the one question that was going to be the clue to reaching the primary DD. maybe i ws just hallucinating....!!

I also hear a lot from people that the patients seemed satisfied with their performances. Even one of my friend told me that a SP told her: Doc I really appreciate you. So I started thinking that maybe I'm gonna fail in my exam because the SPs for me were totally neutral, no negative or positive emotions at all. They just answered my questions and whenever I asked any thing else they said nothing! No more information to open ended questions!

I also took my exam in Philadelphia. Maybe centers are different?!

Nooo, relax Nadia! I think they are supposed to play like this! Moreover I think they are different actors and they have different roles every day. Don't forget - THEY ARE ACTORS but we should behave like in real life!! I took in Philadelphia too. I think one of the big "problems" with this exam is because of US!! We forget to be ourselves , to look at that patient like would be our patient in the office/ ER. It is an exam and we are stressed but the patient doesn't have to know/feel this! I my real life I was concerned many times about a patient but I tried to talk with him/her like he would be a friend and to tell, honestly, all I know and all I can do for him/her! Nadia, you will gonna be fine, you will see! I know it is hard but try not to think about the exam until the result will come!

congratulations on your exam, NE and i absolutely agree with you - SPs are actors, but to us they are real people and should be treates as such. unfortunately, i was so nervous, that i forgot all about it once in the room. and that will cost me alot. have to wait until sometime in october to see how much....and for now i keep counting my mistakes, and list seems endless..... good luck to you in residency match

No more information to open ended questions! i think that they are playing as real patients, so some of them smiling, some say that they really appreciate, some are concentrated on their problem only, but how we know is it their play or not. It is easiear to deal with smiling old lady and not that easy with teenager, but there are nice patiens and there are not that nice patiens in reality. So my guess that all their smilies are not the hints that one's performance was good, but only scenario. Just guess though, don't know for sure.

When I practiced at home with my preferred "patient" ( my husband) I used to finish the case in 12-13 min. There everything was different- for the first patient , when I heard "You have 5 min left.." I was stunned because I DIDN'T EVEN STARTED physical exam and was a NEURO case. After the exam I had a BIIIIIIIIIIG list with what I forgot but I passed. Nobody asks us to ask all the questions, to write down all the differentials or to do all the physical exam. I think they want to know if we know HOW to do them! It is just my opinion.

Congratulations for finishing it! You will definitely pass! Because you are a real doctor. I guess that is what CS really tests.
