80 yr old elderly man in residential home, refused to eat and drink in the last 2 weeks. He had h/o depression and had ECT in the past. He thought somebody put poison in his food.His PB, HR and other investigations were normal.
What is the diagnosis?
1. elderly depression
2. depression with psychotic feature
3. paranoid schizophrenia
4. confusion
5. bipolar disorder

2. Depression w/ psychotic features
He has h/o of depression, being depressed for 2 wks w/ paranoia

I agree. Unlikely to develop schizo at such late stage of his age.

Even this would be a case, 2 week history rules out this diagnosis...
