andrew leipus 29 years old male was brought with compliants of severe lethargy and tired ness . he admitted to eating in non-hygeinic restaurants and subsequent episodes of bloody diarrhoea. he also added of lack of appetite and difficultyof breathing on exertion. on examnation, patient has pallor, spoon shaped nails, but otherwise noremal. he confessed that prevoiously he was diagnosed as paranoid schizophrenia, but as of now he has none of it. he does not seem to have hallucinating, and he does not have any delsions. his haemoglobin is 5 gm/dl. physician has informed that he needs blood transfusion. at this point of time, andrew informed that he is a jehova witness and does not any of this to cure his anemia. what do you do? a. treat him as he is clearly incompetent to refuse treatment b. honour his wishes c. refer to hosipital ethics committee d. refuse to treat him e.ask him to change his religion

very good but...tough ----question, I think...I will go with (a). treat him as he is clearly incompetent to refuse treatment ...

the correct answer is B. HONOUR HIS WISHES. a diagnostic label does not mean anything. apart from that, there is nothing to suggest psychotic about the patient now. and importantly, competence is NOT a medical issue. it is a legal issue.COURTS DECIDE COMPETENCE. as a guidline , on answering USMLE ethics questions, ASSUME COMPETENCE, WHEN IN DOUBT. there is no middle ground here. so, the answer is a clear cut B.
