Click on International Medical School Graduate Application (Revised 12/05),
That is the california letter real name PTAL (postraining authorization letter)
Its a real nightmate but if you have the money, time and SSN go ahead and do it but remember it takes up to 9 m for the processing time.
It allows you to apply and be a resident in any program in the califonia area if you are a graduate of a school outside US or canada. They make sure that you studied all the subjects that they want and during the times they want, if you didn't they make you take remedials. Good luck with that nightmare!!!
