In Acromegaly:

Best screening test is IGF level

Best diag test is Oral Glucose tolerance test

TTT--->Bromocryptine (25% response)

Octriotide (80% response esp in macroadenoma wich is the most commean cause)

Surgery for refractory cases

Is This right?

Step -up to Medicine:
treatment:
1-surgery(treatment of choice)
2-Radiation therapy if no response after surgery
3-Octreotide to supress GH secretion

Medical treatment is indicated if Transphenoidal surgery not possible!

medical treatment is

octreotide
bromocriptine or carbogline if above doesnt work
pegvisomant if both of the above dont work

But with treatment the tumr can get smaller then it will better to do the surgery.

So isnt tx the first thing with surgery in the future after tx?
