Most likely A. bipolar

The psychotic symptoms are mood-congruent (grandiosity). I don't think this is shcizophrenia.

I think this is substance abuse

expl:

the pt already had depression at the age of 19 (10 years ago so unlikely to be Major depression, m b Dysthymic if she was still depressed), but she has both manic(working 3 days non stop) & psychotic features, & these symptoms r for 3 days only--->Substance abuse

pt may have mania sx but she is having auditor halucinations:hence dx is schizophrenia

schizophrenia is not diagosed before 6 wks of symp, if <6 wks--> its schizophreniform disorder..

well can't be that either for 3 reasons:

1, the pt is hyper functioning, not depressed.

2 she had depression at the age of 19, i e 10 years ago, dep > 2yrs is not called depression anymore, its Dysthymic disorder

3. Her sympts r there for 3 days & u need atleast 2 wks of sympts for BPdep

Manic episode is defined as at least 1 wk of elevated mood, characterised by reduced need for sleep, grandiosity ("You are going to be Virgin Mary"), etc. If you are hospitalised, the period of elevated mood is irrelevant - the pt is deemed to have a manic episode. This pt has not slept for 3 days, she needs to come into hospital.

This could be substance-induced mood disorder, except that there is no mention of it in the question stem.

Ok, this pte has Bipolar disorder, but Is he in manic episode or hypomaniac? What kind of bipolar disorder is it? grin


answer:
Hypomania is usually less than 1 wk (at least 4days) , and mania is more than 1 wk and it is more severe form.

Bipolar I = mania+ despression
Bipolar II= hypomania+ depression.
