In 46XY, when r we supoposed to remove the gonads..

-->UW, on diagnosis for fear of malignancy(Gonadoblastoma), + give growth hormones

-->Kaplan, at 20 yrs of age for development..

Which one is it??

20 yrs or late teenage years preferred toallow pubertal development to occur spontaneously with production of estrogen from aromatization of the high levels of testosterone produced without hormone replacement

The early orchiectomy was indicated in the past. Recent studies however indicate that there is a very low risk of testicular malignancy up to 18 yrs of age in pts with androgen insensitivity sdr and recommend the surgey after 18 y/o. I would go with the Kaplan choice, but this is just my decision.
