UW says)The decision of using N-acetyl-cysteine for acetaminophen overdose is based on the drug levels taken at 4h post-ingestion. But if taken>7.5g acetaminophen or levels are not available at 8h of ingestion=>start treatment NMS says U should give loading dose of N-acetylcysteine and maintenance dose 4 hourly,then do check the drug level.If in toxic range hospitalise and investigate. Pls what do U think should be done first give N-acetylcysteine or wait for 4 hours to get the blood level in a situation where the presumed ingested dose is unknown Thanks

In Acetaminophen toxicity, the one of the first thing that u have to do is measure Liver Function Test. Also you need acetaminophen level in serum to begin NAC. Remember that NAC is not helpful if given after 8 hrs of ingestion.
