Medicus_81! I just had my Step 1 yesterday and feel the same exact way. I had lots of Immu, Embryo, Onco, Genetics, Neuro and Behav. Sc., or at least, that's what I remember. During the exam, I was amazed that they could ask me so much 'little subject'-crap, instead of focusing on the bigger things like Physio, Bioch. and Micro. Right after the exam I felt good, because I was SO HAPPY to be done with that bullsh..., but last night I slept so bad, because I started remembering the tons of Q's they asked that totally left me clueless or guessing. Now, I'm not sure if I passed. I feel miserable. I had the impression, that there were quite a few 'easy' Q's, but that the other Q's that I was familiar with the content, were asked about from a totally different angle. Don't get me wrong, if you understood the concept during studying, you're probably able to solve the Q's, BUT I felt you don't only have to totally change your view on the subject (which is a hard thing to do during an exam), but also twist your brain and arm to hopefully get the right answer. I don't know if that makes sense. It's hard to explain. I'm an IMG and studied full 3 months for the exam and felt quite well prepared. I studied with Kaplan videos, FA, FA Cases, USMLEWorld and Kaplan Q's. I also did NMBE 3 10 days prior to Step1, which made me feel very miserable too, but I ended up with a score of 510. Today I feel very bad and all night long all the hard Q's kept flooding my brain and I suddenly realized that during the exam I tried to suppress those, in order to keep my spirits up and keep going. Now I'm totally confused and anxious. All I can do for right now is keep PRAYING and PRAYING. I don't mean to discourage anyone, but it IS a tough exam and they definitely ask you known stuff with a big twist, which is discouraging at times. And honestly I don't know how one could prepare for that type of Q. I mean you study so much and learn 99 little details about every subject and they'll go ahead and ask you the 100's thing, I don't get it. Don't get me wrong, I believe a little challenge to test your knowledge is fine, but some Q's were like: OK, I recognize the letters, but I don't understand the meaning of the words. Totally like a different language. Oh, SORRY for sounding so negative, but that's how I feel right now. Maryam.

To medicus_81: What about cell and molecular biology?, Can you tell what topics were tested? Please talk about the question format, were they vignettes, experiments, etc, please give a sample, not necessary from the exam. To Myriam 187: what do you mean with 'little subject'-crap"? Is kaplan enough for immuno? Also please tell something about the questions format and HY topics, not content, just say for example: renin-ang-ald is HY, etc... This can help an is not illegal.

@Maryam187: I totally agree with you- I felt quite often that I am familiar with the topic but the way they asked about it was sooo twisted and went into sooo much detail...I feel with you! Even now- more than 10 days after the exam I still get anxious and question the possibility of passing...But what can we do? Stay positive-it's easy to say but you can't let this exam ruin your spirit. The whole process with USMLE, ERAS is like a marathon- you gotta save your strength for the long run! Let's keep the faith and hope for the best. @Grandmaster: know how to recognize cell organells and gene recombination, but I think that everybody gets a different emphasis in the respective exam... good luck to everyone!

Grandmaster, by 'little subject'-crap I mean that I studied for example SO MUCH Biochemistry and Micro in DETAIL, every freaking little word basically, which as you know is a BIG lot of info. And they asked very superficial stuff in those subjects, but asked detailed knowledge about embryological origin of let's say 'the cells that activate melanocytes in the skin' or something weird like that. Might be an easy Q for some people, but I find these type of Q's disturbing. Or instead of giving some GFR, RPF, RBF, FF or God knows what kind of calculation, they gave me 2 reactions with their reaction energy, something like 'A->B Diff.G -1.4 and C->D Diff.G -0.4, what is the energy needed/provided (don't remember what) for reaction A->D'. I mean honestly, if you've focused on that type of stuff, it's probably a very simple Q, but as I said, I focused on the very big, big, medium and small subjects, not on things, that have been mentioned once for 15 sec in the Kaplan videos and don't even appear in the FA. Or, I'd say I'm pretty good in Patho, esp. recognizing neuronal structures and knowing their functions, etc. But they gave crappy pics of weird looking cross sections of the brain, where none of the structures seemed to be clearly visible. I honestly think, that's very unnecessary to ask a Step 1 student to recognize structures on bad MRIs, that's already a challenge for a radiologist. I mean, we learn so many details and I think it's already a big accomplishment to know a minimini-detail about pretty much everything they could ask you, but they ask you a nano-thing. In Pharma, there's so much to know with Antibiotics, Antivirals, Antipsychotics, Antidepressants, Chemotherapy, etc., but they asked about very uncommonly mentioned drugs for chemotherapy and their consequences on this or that. Please don't get all this wrong, there were quite a few very simple Q's and after the exam it's probably normal to only remember the bad stuff. Overall, as I read through the after-exam-experiences, everybody seems to feel more or less the same way and many people get great scores. My score could be anywhere between just passed (hopefully I pass at all!) and A+. It all depends on how many 50:50 Q's I hit correctly, how many not counting test Q's were in there and how many Q's I was totally clueless about I accidently hit correctly. Best of luck to everyone!!! My whole body aches and I'm only slowly regaining my spirits. Maybe all this is just very hard for me, cause I finished all those bull crap exams in Germany and now have/had to get used to a whole other system of asking and answering Q's. I love challenging Q's that tickle your brain and help figuring out, if you really understood how things fucntion. But I'm NOT a big fan of discouraging Q's that I feel I could never be prepared for or be able to answer even if I was allowed to use the FA. I'll hopefully feel better and more positive tomorrow and will make sure I send out some positive vibes to you pre-exam guys! Maryam. Hey Maryam! Where did you go to school in Germany? I just graduated in Nov 06 from Universitaet des Saarlandes and also had to deal with the same bull crap exams like you ))))) Are you going to participate in the 2008 match?

Hey Medicus, I graduated in May 06 from Charite Berlin! I don't think I'll be able to participate in the 08 match, cause I still have Step 2 CK&CS ahead and time is running out. But I decided not to stress myself and prefer studying well in order to get some good scores. We'll see if all the studying helped for Step 1. Oh boy... I plan on passing my exams as fast and good as possible for me and then do some internships, which are always a good selling point in your resumee. Wo lebst du eigentlich? Ich bin in Greensboro, NC. Maryam, das ist echt eine schoene Uberraschung! Ich lebe zur Zeit in Ohio, ziehe fuer ganz kurze Zeit nach Kalifornien, suche aber einen Job in DC )- alles der Liebe wegen. Ich habe Step 2 ck&cs schon hinter mir, lief eigentlich ganz gut, warte jetzt auf das Ergebnis von Step 1 und dann bin ich hoffentlich ein fuer alle mal fertig mit dem Zeug! Ja, externship ist seeeeehr wichtig. Ein Problem in deiner Situation waere, dass du keine Studentin mehr bist, du bist in der grey zone- keine Studentin mehr, aber auch keine lizensierte Aerztin. Externship(hands on experience) kann man eigentlich nur als Studi machen. Danach geht nur observership- quasi du guckst nur zu, darfst Patienten nicht anfassen. Die KHs bevorzugen externships, aber observership wird dir seeeehr helfen um einfach das System zu verstehen, ist hier naemlich sehr unterschiedlich(ich war 2 PJ Tertiale hier in den USA und hatte am Anfang ziemliche Schwierigkeiten-ist aber definitiv schaffbar!). Also viel Glueck, wir sind im gleichen Boot und warten auf Step 1 Ergebnisse....Lass mich wissen, wie es war, ich werde mein score auch hier posten. Und frag mich wenn du irgendwelche Unklarheiten hast!
