I think it should be either B or A. The only point against B is that he still has Hallucinations after increasing the dose of the medicine.
I have not come across Lithium causing Extrapyramidal reaction. If you have seen it anywhere, please mention the reference.

The typical antipsychotics (eg haloperidol) cause dystonic reaction. The atypical ones (eg clozapine) tend not to do that, but has serious adverse effects like agranulocytosis.
