Should the photograph you submit to ECFMG/ERAS be of your face only or face and shoulders?

and once i have uploaded a photograph is there a way to know that it has been accepted by the ecfmg ? because somewhere the ecfmg site said "If your photograph is rejected, you will need to resubmit it "

I think they tell you right away after you submit it if the computer rejects it or accepts it.
