In kaplan they r saying not to give RhoGAM if sensitization already took place??, ur too late they say, is this right or we should still give RhoGAM??

In the next pregnancy, still don't give or the Anti Rh abs r not chronic..& if so, how long do they stay?

rhogam is anti D. it is given to rh- mothers with rh+ babies. the rhogam destroys the D and prevents an immunological response, were rhogam not to be given then mom would make antiD herself which would then attack her rh+ baby and hence all the complications that go with a rh- pregnancy.
.the idea behind rhogam is to give it fast enough before moms body has had a chance to set up an immune attack against babys D, hence the 72 hr deadline foll delivery or any kind of antepartum bleed or even ectopics , abortions , MTPS, basically anything where the Rh- mom is likely to come in contact with a RH+ blood .also given to be on the safer side at 28wks
once the mom's making her own anti D the purpose of rhogam is defeated. then all you can do is damage control. caefully and regularly monitor the bby with amniocentesis ,plotting the bili on a lileys chart and when and if bili enters zone 3, decide regarding inutero transfusion or delivery dep on fetal maturity.you can also do a PUBS and look a the hct and transfuse if hct falls to below 25.
