Mother of adam tells the problem of her 8 yo M child that c/o nocturnal bedwetting. Started 3 yrs ago ,happens 3 times a week. No dysuria ,no urgency ,no changes in color/quantity of .Normal Fluid intake before sleep. No hematuria. No constipation. No fever/chills/sweats.No sleep apnea .No nighttime awakening. mother says no stress in child's life. No environmental changes.good relations with family and school friends. Tried to treat it with night alarms,but stopped them. No neurological problems (no numbness/weakness/headaches/visual problems. ROS : negative except above. PMH : None. PSH: None. Allergies: NKDA. Family Hx : no similar problems. No changes in bowel habits..
Birth history : Full term ,C-section delivery. Healty when born. Up-to-date with immunization schedule. Pediatrician : Dr.Smith - Telephone number:99999999.

PE Not performed -

Primary Nocturnal Eneurisis
Secondary Eneurisis
Constipation
Urinary Tract Infection
Functional Bladder Diseasev

Genital Exam + General PE
Urine Analysis
Urine Culture
US- Renal
CT- Abdomen

Don't forget to include in your Ped's Hx ( after the Birth and Vaccinations) :
1- Mile stones : his "Last checkup" was ...wks/months ago ,and was WNL for his age
( it's better to be more specific if you have the time write : Ht , Wt , Language ...etc , are all WNL for his age )
2- Nutrition : Breast milk/Formula milk/Solid food / prescribed Food(like Vitamins or Glutin free diet, ...etc)

Here's a good Mnemonic for general Ped's Hx ( i saw it somewhere i don't remeber) , combine it with PAM HUGS FOSS {of corse not all of it , no sexual or social hx expected! grin } and u should be on the safe side !

BJM CHILDRENS

Birth Hx
Jaundice
Mile stones
Check up ,Last / Crying
Head complaints : e.g Meningitis, Siezure
Immunizations
Lethargy
Diet / Daycare
Rash (Description , Distribution ,Relation to fever)
Ears & Eyes complaints
Nutrion / Nose complaints
Sick contacts
