What are the Indications? Contraindications?

Indications:
Chest pain, and EKG normal.
To Confirm Angina
To determine severity of disease
Post MI eval (?)


Contraindications:
Any reason for Abnormal baseline EKG
or patient can't exercise (obese, etc)


(in order to do an exercise stress test, you must have a baseline EKG and the patient has to be able to exercise).

Anything else?

1st of all the CI of Exercise Stress test r:

Young Female (high false +ve)

Wolf PW Synd

LBBB

MVP

Acute ECG & drugs affecting the test ECG ( Digitalis, Quinidine,...etc)

Do a Thallium/Persantine Stress test, or adenosine stress echo

Absolute CI for Stress testing:

3 Aorta, 2 Acute, 2 un(........)..:

Aortic Dissection, Aortic Stenosis, Heridit subaortic stenosis..

Acute changes in ECG, Acute CHF (or uncontrolled)

Uncontrolled HTN, unstable Angina..

Pts work may may risk public health (bus driver, etc...)

GO STRAIGHT TO CATHETER....

What about indications? Any other indications besides the ones I mentioned... I also had a question about post MI being an indication -as post MI, there will be EKG changes which will interfere with the test.... any ideas?

In post MI, the changes r not acute anymore, u don't just stabilize the pt & discharge him, moreover u need the stress test to know the degree of defect in cardiac funcs, so as to add a defibrillator or not( if CO < 35%)

another indication is fitness for exercise in >=4th decade pts if they r asking about it (not indicated as a screening test)
