Extra-Pyramidal Adverse effects of antipsychotics:

1. Acute Dystonia: Early, ms rigidity, occular crisis, Torticollus

TTT---> Anticholinergic --> Benzotropines & trihexyphynadil

2. Acthesia : Can't sit still (antz in my pants)

TTT---> BBs

3. Tardive dyskinisia: Lip smaking, chorea

Prophylaxis: Benzo

TTT: Bromocruptine or Dantrolene

4: Neuroleptic malignant synd: Irreversible condit

Hyperthermia, ms rigidity, etc..

TTT---> ICU, CK level, Stop drug, Bromocryptine or dantrolene

Specfics:

Fluphenazine---> hypothermia (d t inhibition of shivering & Vasodilat)

Ziprasidone: no weight gain

Olanzapine: Weight gain the most

Incompliant pt : Decanoate form ---> Haloperidol (1st line, every 4 wks) / Fluphenazine (2nd line, every 2 wks)

is that it???

MAOIs interactions:

1. with tyramine---> hypertensive crises.

2. with SSRIs-----> Seretonin synd--> hyperthermia

3. with dextromethorphan (all over counter cold medicats)---> hypertensive crises

SSRIs:

Aggitation: Don't take before goin to sleep

Sedation: Don't take during day

Sexual: Use 1st Bupropion, 2nd Citalopram

ADHD developed depression : Bupropion is the drug of choice

Priapism: Trazodone

Buprpion: Decreases seizure threshold, Monitor BP..

I thoght TTT is swicht current antipsychotic for low potency or atypical antipsychotic. Input pz

This is not antipsychotic but

METHYLPHENIDATE (S effects).............source UW

1.-Nervousness

2.-Decrease appetite

3.-Weight loss

4.-Insomnia

5.-Abdominal pain

*Should not be used in children less than 6 years

CLOZAPINE......source kaplan notes

1.-5% seizures

2.-1% Agranulocytosis

3.-weight gain

4.-Drooling, sedation & anticholinergic symptoms

*Schizophrenia, second line medication

MAO interactions

HYPERTENSIVE CRISIS:
Tyramine (cheese, red wine)
TCA/Bupropion
Sympathomimetics (i.e. pseudoephedrine in cold remedies)
Guanethidine, reserpine, L-Dopa

SEROTONIN SYNDROME:
SSRIs
Dextromethorphan (over-the-counter opioid cough supressant)
Meperidine (opioid analgesic)

Clozapine

clozapine is the drug of choice for psychotic patients with sucidal attempts or

agressive behavior
