5 yearold boy who immigrated to USA need a shots.........mother has lost the immunizaton card given by health care ..... but she has a proof of written immunizatin paper by herself.......... what next .

a. consider baby as sussceptible for all immunization
b. valid her written paper as prior immunization

Do we need official health care provider record of immunization or is Mom's written notations acceptable here in US???

Vaccination is not an obligation. Parents can choose for vaccinate their child or not, because it is not life threading situation we do not need to reinitiate whole vaccination schedule.
Also pt's mom brings information; I don't see any situation to mistrust her intention.

nothing abt mother's intention given.... so cannot say mother is doing it purposefully but as kid is IMMIGRANT i wud give him all shots for his age ....

official written papers are required .... my personal experience..... used to get all shots regularly back in home country ...but those are not valid as i have no written official document ..so have to get all shots again ....its their rule ...

I don't want to get personal, but all of us who are from other countries, we are forced to take the shots because we are going to be a healthcare provider, if you choose to do something else you won't be forced to take the shots again.
I think the answer is B, there is no reason to mistrust the mother.
