In asymptomatic obese adult with family history of obesity and HTN.... If we are screening for lipid profile, it should be first the fasting lipid or the total cholesterol (isn’t it also fasting)? What is the difference between fasting lipid and total cholesterol? I though it was same ?!?!
 Could some help me here~ Thanks.

FASTING will have less quilomicrones wile the othe one will have a TINY little more. But actually Goljan said that affects more the TG and not the total cholesterol. WAO even here Goljan still hunts me I THINK, if not please correct

the screening test is total cholesterl ..... fasting test has lless TG.

Here is one of Q of UW that I found today... 45 yo male whose friend die CAD, comes to your office concerned about his health. You decide to screen him for lipid disorder. What test are you going to order? A) Total cholesterol B) HDL cholesterol C) Total and HDL cholesterol D) Total and LDL cholesterol E) Triglyceride F) TG and Total cholesterol G) Etc.... ??? Answer said that is C) Total and HDL cholesterol and one of the Pediatric Q in which boy has obesity, they said screening with total cholesterol. So I'm confused.... Could someone explain me why ? Thanks again.

Elitoki, did they mention in the first question about the Obesity? or just the boy was obese? & what was the entire explanation including why others were not the probable answers?

This was one Q of pediatric but I’m posting here to compare the answer between adult case and child case…. Obese Caucasian father brings his 10 yr old obese son to your office. The father states that he was recently diagnosed with high cholest level and CAD. Even though his son is asymptomatic he is worried about the obesity. What is the next best management? A) get a fasting lipid profile B) get a screening test for total cholesterol level C) advise low chol diet D) evaluate him for CAD E) Advise to start a regular physical excise schedule. Correct answer was B. So, again my Question was why we have to screening in Total cholesterol and HDL in adult and in child just total cholesterol? When we ask to the Lab for total cholesterol they usually inclued HDL? I'll apreciate your answer~

Well screening recommend r: Total cholesterol & HDL cholesterol..starting at 35 in M & 45 in F ( start at 20 if +ve RFs) TTT depends on LDL 1st.. In the scond q ur not screening for CVD RFs as this should only be done at 20 years of age, ur investigating for his obesity cause..
