Kaplan Qbank USMLE

I wasn't able to copy it, but here goes

a pt presented with chest pain radiat to left shoulder, the resident got an ECG while he was giveng him nitrates, the ECG was normal & after he took nitrates, the pain went away..What do u think the diag is?

1. GERD

2. Achalasia

believe it or not the answer was GERD????

From what i understand,in Achalasia, nitrates help the LES to relax so the spasm would go away (same as in diffuse esophageal spasm), But i really can't get its role in GERD?? i thought may be a stricture, but then thats fibrous tissue, not muscle, so wouldn't relax on nitrates??

Can someone clarify this? Appreciate it

It's an obvious mistake, and you man are right... Relaxing sphincter with nitrates (as well as anticholinergic drugs, antispasmodic drugs, many anti-histamine and antiemetic drugs, tricyclic antidepressants, phenothiazine neuroleptics, calcium channel blockers) would aggravate GERD...

Justice is right , nitrates will worsen GERD obviously.

I just downloaded it a couple of days ago, its good (more difficult than Kaplan Qbank, but easier than UW)..

dear citrus.... i guess this is amistake.

insteed f GERD the correct answer is DES ( diffuse esophageal spasm ) which gives a picture similar to angina and treated by nitrates.

it's surely a mistake, cirus,
agree w/ dr.wad. Diffuse esophageal spasm presents w/ symptoms similar to angina, non-contributory ECG and symptoms relieved w/ nitrates.

Yes, Thats what I know but since DES wasn't there, my second choice was Achalasia
