In AF TTT

1st : duration of symptoms:

a. < 48 hrs

TTT--> DC Shock

b. > 48 hrs

TTT---->

I) Hemodynamicaly stable:

Medical rate control ( 1st BBs, 2nd Ca chan Bs, CHF Digoxin) or we don't control rate here??

Then according to age & RFs :

Less than 65 yrs & lone AF--> Aspirin

Age more than 65 & / or complicated--->warfarin + Heparin ( esp with high vent response)

wait 6 wks on warfarin--> DC

II) Hemodynamically Unstable: Heparin & DC

c. > 6 months

Medical rate control..

Is that it???

atrial fibrillation
1. hemodynamically unstable= cardiovert

2. hemodynamically stable=
a.)first rate control= digoxin/beta blockers/CCBS
anyone of above dep on pt profile and assoc features
b.) then rhythm control (chemical cardioversion)= ibulitide

3.) atrial fibrillation present for more than 48 hrs , first anticoagulate then cardiovert. if cardioversion urgent r/o clot with a TEE

4.) atrial fibrillation as the lone symptom in a young pt with absolutely no other personal history or family history of CAD, HT ot other CVS risks then aspirin alone.

dr arc, I think u mean Rate Control 1st - BB, CCB and Dig prefferd in Pts w/ CHF of LV dysfunction i.e. low EF

Sinus Rhtym control is w/ antiarrymt as u said Ilbutlitlde.....

In Gnereal any pt that is hemody unstable
-sedate 1st than DC

< 48
- control rate 1st

> 48
- Control rate 1st along with anticoag
- Rhym control if needed
----either TEE to r/o clot, if negative Antiarrh (chemical conversion)
----If TEE not done antivcog 3-4 weeks prior to chemical conversion and anticoag afterwards as well
