For marijuana - every time, it's illegal in US, and may result in DUI... For alcohol - only 2/4 CAGE Qs are "yes"... For tobacco smoking - I would just ask if the Pt has ever considered to stop this practice... Won't hurt...

DUI stands for ?? duodenal ulcer ?! abt alcohol i have read a nice trick ... no matter how much he drinks ,ask him/her : do u get into any trouble or have any problems after you drink ? if the answer is yes ------) CAGE immediately Best of luck guys

always counsel the patients about the social history, and whenever they said yes, i would ask if they ever wanted to quit, and if so we can help you with that later. otherwise we can talk about it next time...

I second what damselMD says. Just don't forget to counsel when you are under pressure, take a note that you should counsel or counsel on the spot.

If SP says. i smoked for 5 years but now stopped ,not smoking anymore. would u counsel this SP?

As a general rule I wouldn't. There is no need to. It's always good to ask when the patient stopped smoking, the patient could have stopped smoking 2 days ago, I think it would be wiser to counsel a patient who just stopped smoking rather than a patient that hasn't smoked for over a decade; the latter has probably already been through counseling.

drug use? .. if yes .. counsel, offer to help with support group, and offer to contact social worker alcohol use? If yes .. CAGE .. if yes to any of the CAGE questions, then counsel, offer to help with support groups, and offer to contact social worker smoking? If yes .. Counsel .. offer to help with support group. If stopped, ask when, and offer positive support. always consel, it doesn't have to be elaborate .. "sir you should really stop drinking, it's having a negative effect on your health, your work, your family, and can lead to legal problems in the future. If you would like I can set you up with a social worker, to help you get through this." and don't forget to document this in your note.
