what is it about these qns? either they're so easy that i ace them , or they're insanely hard. what about the regular qns for the regular joes? well, maybe i find the easy ones easy coz i answer them right. i guess i need to start working on the tough ones. [maybe they're tough just for me? hmmm....there's a thought!] each person has their strengths and weaknesses. i need to make my strengths stronger and pull up my weaknesses BIG TIME . no way out! i have to survive in the Land of Tough Stuff!! yep, that's where i'm headed. gotta slay 'em. hmmm...back to some more qns. now, where's my sword?

under normal circumstances, ventilatory drive is maintained by central chemoreceptors, by sensing PCO2. these receptors are slow-acting, and adaptive. periferal chemoreceptors respond to drastic reductions in PO2. they also respond to sudden rises in PCO2, when reaction by central receptors will be too slow for survival. at high altitudes, PAO2 and PaO2 are < 100. here, main drive for ventilation is drop in PO2 via periferal receptors. O2 dissociation curve: L shift [causes tighter bonding b/w Hb ad O2, hence O2 not easily released], seen due to: 1. hypothermia 2. dec 2,3-DPG 3. dec H+ 4. dec PCO2 5. HbF R shift [causes less affinity b/w Hb and O2, hence O2 easily released], seen if: 1. hyperthermia 2. inc 2,3-DPG 3. inc H+ 4. inc PCO2

the kidney consists of glomeruli, arranged in series i.e. high resistance. [for effective filtration] capillary hydrostatic pressure is the main force responsible for the filtration. plasma proteins are not filtered, due to negative charge present on glom basement menb [GBM] hence, conc of proteins is higher in eff arteriole. this helps in inc plasma oncotic pressure [reabsorptive force] in the peritubular capillaries. [for effective reabsorption]

hi there, simi. thanks for coming by! all well with you? hello, doc499. welcome to my journal. i see that you've recently begun your own journal. good move, it's help you and keep you motivated. will drop in soon. p4a99: keep these gems coming. i love the challenge. hope you're doing good. nothing wild planned for the weekend, i hope??!! i was in for a big shock this morning as i settled down with my kln, in the mood to tackle renal phys. things were going well, right up to the time i reached reabsorption. and i couldn't for the life of me remember what gradient-time was!! i mean, i didn't even recognize the term!!! i was like, WTH!!! have i seen this before?? and there was all my notes added in there, stark proof that i'd walked that path before. it's maybe ok to forget details of things, that's why we have re-reads. to revise, refresh, to master. but, i can't possibly fail to recognize a term entirely!!! this can't be happening, not now!!! actually, not ever!!! hope it doesn't happen again! these things only ever happen to me!!!!! not a good sign!!! well, back to the grind, now.

don't you worry Paheli, it happens to all of us!! You are doing very well!! Keep up the efforts on SWOT analysis: analyzing our strengths, weaknesses, opportunities, and threats and working on them is sound planning!

i have zillions of such moments. the problem with my memory is like a double edged sword. on one edge, if i see unremarkable terms like these - gradient or time - i absolutely cannot put them into context. we know what is gradient and ofcourse time (OMG!) but "gradient-time with respect to renal physiology" is not sufficient context to recall the concept. so i think i should attach more context to it next time i learn about it and memorize both the term and context together. more complex and unique contexts so that next time i see such mundane day-to-day terms i don't just blank out. the other edge, when i see really unique words especially all those eponyms - i blank out. here the problem is these terms are absolutely unique (which, if you notice, is the solution to the first memory problem) and we have to attach some mundane day-to-day term to this eponym and memorize it, to aid in recalling the information about it. its all about the tricks your memory play to fail you and how you recognize them and charge back with overriding tricks to subdue your failing memory!!!!! Hope I was not vague!

lol!!! yeah you were, bioguy!!! but, it made sense to me, too!!! so, we need to out-trick our tricking memory!!! well, it's good to know that i'm not alone. thanks for the support, acintya!! nice to see you again! another thing i've noticed is that in some wierd unexplained, maybe unexplainable way, my performance yo-yos all over the place. i miss the freebies in one block, and in the next one, i do great! so, i can never quite figure out where i'm at. and, if this pattern continues, i'll be manic- depressive before my test comes along! lol!!! God help me!!!

hieeeeeeeeeeeeeeee! Awesome stuff. Why weren't u taking the exam with me. Such good updates here. Keep it up. Hey I don't know about me, but if you keep revising memory does stick. Revision will be your key. Don't worry about the questions. The more experience you get with them, the more you will be able to tackle the absolute insanely hard ones. I am so happy to see you have picked up pace. Keep it up. Good luck to you.

Helloooooo How are you? Cuz youre doing Qs these days, i thought this could be of a little help.
njc... wrote:Hi praying4a99, thanks for dropping by. About using Qbanks (both Kaplan and UW, not only UW for me), solving the questions is not the key issue here. I would do them quickly, (most time could not care about the score), but spend the majority of time in reading all the explanations, making notes. From every Q, I learn a lot of stuff. Even the question itself will give me a lot of insights. I review at least two times, more possibly three times for the Qbank. Then at last, I would do the marked Qs one more time, randomized, timed mode to test whether I have really mastered most of the concepts (the first time, subjects-wise). I gather at that time, I could get at least over 90 for each block of questions. For step 1, I just used the Qbanks and FA, nothing else. But for step 2, I would add kaplan notes to my study formula. But I would only go through kaplan notes very quickly for two times, the most. Every one has different approach to get a 99. Make sure your route will help you master most of the important concepts of all the subjects, and build up your skills in thinking and solving problems in a quick and right way. Hope this will help you a little bit on your preparation. GL.
njc... wrote: one more thing to say. When reviewing the Qs and the explanations, I am thinking all the time, why I did that Q wrong, why I did that Q right; if the question stem changes a little bit, what will be the right answer; if the question asked something a little bit different, what will be the answer et al. When making the notes, sometime I put similar things together. For example, under what condition, you prefer cesarean section for a pregnant woman; I just list different situations which need c-section in one page. So it is easy to compare and memorize. If you don't feel this is a comfortable way to study, don't force yourself into it. Whatever works best for you, go with that. We want to study efficiently and effectively, with the specific way we enjoy. Best of luck. BTW she's a 99er. GL

ok, finally, let me get down to some points that i've learnt so far: Pegvisomant: a drug that i'd previously never heard of. it's a new GH receptor antagonist ----> dec IGF-1 ----> due to -ve feedback ----> inc PRL, GH Glucokinase: actss glucose sensor in beta cells of pancreas. hence, in glucokinase def, a much higher blood glucose level is reqd to stimulate insulin secretion. will type up more later, gotta run now!
