Adrenomedullin: a member of the calcitonin family of bioactive peptides. it is hypotensive. first identified in pheochromocytomas. it is expressed in adrenals, lungs and kidneys. elevated in some patients with pulm hypertension. There are 3 defense mechanisms against hypoglycemia. 1. dec insulin levels 2. inc in glucagon level and 3. inc in epinephrine. in diabetes type 1, the first 2 of these 3 are lost. hence, the only warning that patients have of imminent hypoglycemic attack is autonomic in nature via release of epi. the symptoms produced include palpitations sweating, tremors etc Fasting / Starvation: Phase 1 - energy provided by liver glycogenolysis, up to 24 hrs of fasting Phase 2 - gluconeogenesis. duration depends upon body;s glycogen stores and level of activity Phase 3 - ketone bodies. preferred source of energy after 1 week of starvation. Phase 4 - prolonged starvation, whereby skeletal muscle protein is used as source of energy, via ketone bodies. loss of 50% muscle mass is usually lethal, and the person can usually only survive ~ 3 weeks of starvation.

thanks, everyone, for the kind words of encouragement. medoc: i've seen your spankin' new journal in step 3 forum. nice!! will pop in soon, with a nice journal-warming present. you are in a hallowed league now. neck's giving me trouble looking up all the way up there!! doc499: i will be blocking my date any day now. just need to gather my strength before i do it! anyway, i have done uw before, not all of it though. am currently working my way through kaplan qbank. and it's taking ages. i would highly recommend that you start with the qns as early as you can. coz it's time-consuming and time's what you run out of closer to the end. simi: i do hope that these bits of info help you. i know you're in the final lap now. these little notes of mine are not in any way complete, as you can see. but i'm trying to jot down small facts that we're likely to forget. and hopefully, putting it in a public place like my journal, my hope is it will help more than just me . suggestions for improvement are always welcome! another hope of mine with these notes is to try and keep things in the prep journal thread a little active. admittedly, i only keep my own journal moving, but maybe things here will start picking up slowly. i think too many have left all at once. and, it's definitely palpable. of course, it's to be expected. so, if we work hard, we can move up too! i know i can't wait to finish with step 1!!

I will be in better position to analyze it all.. so wait till I get results .. till then if any specific Qn .. you are more than welcome to ask..

I thought you may like this: "Reviewing this stuff (cramming) without achieving deep understanding at each level (internalizing) is like renting an apartment for a month at a time when you know you’re living in the neighborhood for the next ten years. Why waste your money? Instead of paying just enough each month, it’s worth it to go into debt (time-wise) to own it. Every day after that, the investment pays for itself, but you have to make it that first time. I’m not here to rent the knowledge; I’m here to own it , and every criticism I ever make of review materials is in that light." "I was up till 1am or 2am each night and we were both up and at the library by 8:30am, so they were long days. But they were fun days, because you get to see every puzzle piece again only this time (after two years) you know what the fucking picture on the box is supposed to be. It’s amazing how much everything starts sliding into place. " "I’m not going to question my attitude about the whole thing. I still think the way to prepare for the USMLE is to be enthusiastic for the chance to show the world what you’ve learned and to be excited for the opportunity to give two months to simply “reviewing.” Despite the hard work involved and my inability to articulate it in the form of an application essay, I love science. And despite the transient wish for a cat’s simple life of breathing, complaining, and rubbing against things, I know that none of that is what I really want. This, trully, is the greatest thing I could be doing right now. " "Falling behind means knowing less . Excuse my hubris, but I was trying to be the first to avoid this. I am taking 9 weeks were others take 7. I could have sworn it’d be enough. Parkinson, however, couldn’t care less. I give you his Law: “work expands so as to fill the time available for its completion.” Truer words, truer words. But that’s not an option. I’m not postponing my test to allow time to catch-up. I’m not going to drop other interests like writing about this experience. I’m not going to learn less. Something’s got to give. It’s probably appropriate, then, to introduce a new law. Medical Student’s Law: “Sleep contracts as work expands.”

No it doesnt sound selfish at all. I have benefitted from positive energy and having people work their butt around me was very motivating. I know what you mean. I have promised myself to get started coming monday. this week just too much is happening. So hang in there. Do your bit and you would have not seen the end of me. So tell me what r u doing? how far away from the exam are you> what is your winning strategy? How much time do you have to do what it takes? U don't need to tell me really. Just get your answers.
