Guys, although I dont recall the exact question, nor do I have asnapshot of the question...But there is this Dermat question in NBME 2, which is related to acne of some form in a youn female and what will be the most appropriate Rx.

So what is your opinion on that question, if anybody can recollect?
1. Do you start with topical benzoids?
2. Topical retinoids/antibiotics?
3. Oral antibiotics?
4. do you give oral retinoids?

I had the same confusion too...Question said the 19/F had papules and intermittent pustules. We use topical retinoids only when there is comedones right? When there is inflammatory pustules the drug of choice is topical antibiotics--But the answer was NOT there....So I went for topical benzoyl peroxide and I am sure I got it wrongsmiling face

papules with intermittent pustules and also some H/O scarring if I recollect correctly......

I thought that alll these point to nodular cystic acne, which requires Rx with oral retinoids....

u didn't mention any papules or pastules in ur qs.( this doesn't mean scarring, but indeed a severe form..

but if so the TOC is

Oral Isotrentoin + topical retenoids or ABs

in case of scarring = all together ( but i'm not sure wether t give oral or topical)

cirus, I was referring to the original NBME question. I didnt remember the exact wordings, what I was hoping was that in case somebody who had reviewed these questions would be able to recollect if I mentioned the question. Sorry for the confusion,

So what is the FINAL answer to that NBME question?

ill still stick to my ans as it is the only one that combines topical Abs with retenoids
