26) The 18-year-old son of an Arab Sheik is flown to a hospital in the United States after his local doctor discovers an abdominal mass. The young man had a two year history of weight loss accompanied by production of malodorous stool with excessive flatulence. Duodenal biopsy with histopathologic examination demonstrates villous atrophy and massive infiltration of the lamina propria by lymphocytes, plasma cells, and immunoblasts that do not morphologically appear overtly malignant. Biopsy of a mesenteric lymph node shows node replacement by a similar infiltrate. Which of the following is the most likely diagnosis?

A. Heavy chain disease
B. Macroglobulinemia
C. Monoclonal gammopathy of undetermined significance
