What is the appropriate response of aldosterone...to postural changes -lying down to sitting up?
Normally when is aldosterone higher? Lying down or sitting up?

appropriate response --> Hyperplasia --> Tr. not surgical
no/wrong response --> Adenoma --> Tr. Surgical

Thanks.

well I hope I got your qn correct. I think physiologically aldosterone level should be high once you go from a lying down position to a sitting up or standing up position. and hence its low when one is lying down.

you're absolutely right, doc179
Normal response to prolonged standing-->activation RAA axis-->elevated aldosterone

also you can compare the aldosterone levels first thing in the morning and 4 hours later (8a.m and noon):
NL ppl and hyperplasia: increase
adenoma: decrease

or you can compare blood samples from adrenal veins: one sided increase: adenoma, both sides: hyperplasia
