40 YO M has experienced malaise, fever and a 4-kg wt loss over past 1 month. His serun urea nitrogen is 58 mg/dL and creatinine level is 6.7 mg/dL. Renal angiography reveals right renal artery thrombosis and the left renal artery and branches show segmental luminal narrowing along with focal aneurysmal dialatation. While receiving hemodialysis for a week, he is found to have melena in association with ab pain and diarrhoea. Which of the following lab test finding is most likely?

1) +ve c-ANCA
2) +ve ANA
3) +ve HIV
4) +ve HBsAg
5) +ve Scl-70

1) +ve c-ANCA = Wegener's Granulomatosis
2) +ve ANA = SLE
3) +ve HIV = AIDS
4) +ve HBsAg = Hep B
5) +ve Scl-70 = Scleroderma

Now In My opinion This is PAN = Poly Arteritis Nodosa which P- ANCA positive but it also has a relation to Hep B. Hep B Pts r likely to develop PAN

SO my Answer is 4) +ve HBsAg = Hep B

I was thinking it should be +ve HIV...along the lines of focal segmental glomerulosclerosis....

I wud go for HBsAg... 75% PAN associated with HBV...
