Examination shows hyperreflexia of all extremities ---> Decompress 1st.. if one of the ans was IV biphosphonates, i would have chose it as the next best step over surical decompression, always lower Ca prior to surgery, right??

yes but bisphosphonates take a while to lower Ca, don't they?

well the first step should be IV steroids + morphine (for pain), then IV Biphosphonates(for increased Ca) & then surgery or radiation ( if not that severe as it is the preferred ttt) so following this algorithm, it should be surgery
