it causes the Vasodilation of the Peripheral arterioles. So in three stages it causes the High Output Cardiac Failure. they are 1. peripheral vasodilation occurs, 2. This leads to salt and water retention mediated through the renin-angiotensin-aldosterone system in the kidneys. As the vasodilation progresses, the kidneys detect a relative loss of volume and respond by conserving salt. 3 With the salt retention, fluid is also absorbed into the circulatory system. The resulting fluid overload leads to edema of the dependent extremities.

Which then Leads to the heart working as a severely high workload pump to the required cardiac output needed to satisfy end organ requirements. Parts of the heart muscle undergo overuse injury, which results in the physical symptoms of tachycardia, edema, and high arterial and venous pressures. These changes can lead to myocardial injury, expressed as chest pain.

hi am new to forum n hope our chemistry goes fine. regarding u r quest tyhiamine deff causes accumulation of ketones in the cardiac muscle due to insufficient oxidation of glucose in highly aerobic tisssue like cardiac muscle, cause of insuff oxidation is pyruvate dehydrogenase deff thiamine is a cofactor 4 which,

hye, adding a point here coz thiamine is co factor for important enzymes in oxidative phophorylation.like pyruvate dehydrogenase, ketoglutarate dehydrogenase and ketoacid dehydrogenase ,its deficiency leads to ATP depletion, and less energy and more work load on heart ,hypertrophy, dilatation, back pressure and edema and plus above explanation of dilated cardic failure.
