Why does the pt develops gynecomastia ,testicular atrophy in alcoholic cirrhosis ?

because liver is responsible for the metabolism/conjugation of estradiol ( a lipid soluble substance) to make it water soluble for excretion.Hence, if there is hepatic cirrhosis (damaged liver), estrogen is not excreted, builds up causing gyncomastia, increase cappillary fragility-caput medusae, etc.

agree....

:arrow: decreased testicular testesterone production ....explains testicular atrophy.

:arrow: decreased hepatic extraction of androstenedione .

:arrow: increased aromatase activity...converts testesterone to estrogens (peripheral adipose tissue)
