Hi, The answer for both is SURE YOU CAN!. You can speak with the proper office of the university you are interested (usually Graduate Studies; where you need to send your documents) and they will put you "conditional" so you can start your degree and they will wait for your diploma. For the second question the key point is to find a supervisor willing to accept you. Personally I recommend, if you are considering going for the match, a supervisor who is a clinical staff as well (specially in the specialty in your are interested) and no a pure researcher; in that way you can start connections as well, observerships, LORs, etc. The university will never care in you have research experience, only to pass their requirements (TOEFL, diploma, tuitions, etc) so you need to find the PROPER SUPERVISOR and remember that you can dor esearch in many areas (basic, clinical, public health, education, administration) so you have many options. Finally, if you have problems getting into a PhD for any reason, a trick is that you can go for a master and, after one year, switch for a PhD if you still interested. Hope this help.
