desnond hayne is a reitired cricketer from westinides. he is living his post-retired life happily partying with friends. he has been a smoker since his test cricket days. he complains to the doctor, doc, of late i am finding it difficult to swallow food. he points out to the lower end of sternum and tells that food is getting stuck there. he feels sad that of late he is not able to enjoy his beer too.as he has difficulty in swallowing that too.he has constipattion.he lost 30 pounds weight.what is the best initial test to be done in this patient?
a.barium swallow
b.manometry
c.ph studies
d.endoscopy and biopsy
e.CT scan
f.chest x-ray

Why not endoscopy and biopsy?

Yes Why not Endoscopy as u can get the Biopsy also at the same time as it looks like Esophagial Ca.

A barium esophagogram is obtained as the first study to evaluate dysphagia. The appearance of a polypoid,infiltrative or ulcerative lesion is suggestive of carcinoma and requires endoscopic evaluation.
Endoscopy with biopsy establishes the diagnosis of esophageal carcinoma with a high degree of reliability.
As the question is abt best initial test to be done in this patient....it should be barium swallow.
