vib richard,52 years old was a master batsman in his hey days for west-indies.. he is living his post-retired life happily partying with friends. he has been a smoker since his test cricket days.he used to have burning chest pain whch he used to self-treat with OTC drugs. this was the case for nearly 20 years. he complains to the doctor, doc, of late i am finding it difficult to swallow food. he points out to the lower end of sternum and tells that food is getting stuck there. he feels sad that of late he is not able to enjoy his beer too.as he has difficulty in swallowing that too.he has constipattion.he lost 30 pounds weight.what is the mostlikely diagnosis?
a.squamous cell carcinoma of oesophagus
b.adenocarcinoma of oesophagus
c.barret esophagus
d.gastroesophageal reflux
e.achalasia cordia
f.chagas disease

hi ayesha, this is clearly oesophageal carcinoma. the patient is a westinidan, (african , which in layterms, black) male, and then he has risk factors like smoking and alcoholism. he has gastrophageal reflusx also. and then he has progressive dysphagia, both to solids and liquids, and then there is documented weight loss. how can you diagnose barret's oesophagus in this?

u are right, actually i read all three questions posted by u simultaneously and i thought this one has got to have a different answer. I guess i just kindda concentrated on the heartburn part. It was careless of me. Thanks for correcting!! :oops:

also lower esophagus has adenocarcinoma while upper has squamous so it gives clue

pt has long standing history of GERD which leads to Barret's esopahgus which can lead to adenocarcinoma. Plus he has wt loss!
