That's not a BBB question - that's a side of ERAS and applying that I don't see. Sorry. There is someone here that can help you with that.

so- you have a LOR, it has the cover sheet, he mentioned that you waived your right...and you have it in a sealed envelope. The easiest way is to send that envelope directly to ERAS, separately from other stuff. you don't need the doc submission form if it has the cover sheet. You just write the address of the Physician as the sender and submit it to ERAS. Basicly, you just send the LOR as his secretary would, right? so there is no need to go back and request him to do it- he wouldn't do anything different than you now, would he? This is what I would do- easy and uncomplicated.
