I will go with A. The ECG does not show electrical althernans, suggesting the fluid volume is not very big.
The patient is almost hemodynamycally unstable but not yet, blood pressure is not below 90 yet.
I think the patients still can afford to take Echo to confirm the diagnosis first. pulsus paradoxus is not typical for cardiac tamponade.
