30 yr old man fall to the ground while repairing the house roof.He had been unconscious upon arrival at the hospital. A CT scan of the brain shows multiple small hemorrhages and subarachnoid hemorrhage along the tentorium. he is admitted to neurosurgical intensive unit and placed on ventilator. A ventricular cathater is inserted to measure the intra cranial pressure. which is found to be elevated. which of the following mesure is most likely to be used to reduce intreacranial tension. A. administration of vasodilator agent B. intravenous administration of hypotonic fluid C. lowering head to flat position. D. hyperventilation E. mechanical ventilation with positive end expiratory pressure

D. Hyperventilation Hyperventilation is the mainstay of the treatemnt for reducing raised ICT. Hyperventilation by reducing pCO2, causes vasoconstriction in Brain (vasodilation in other organs), there by reducing cerebral flow and reducing ICT. uSE OF OSMOTIC DIURESIS/ Shunts are other vways of reducinig raised ICT.

Positive end-expiratory pressure (PEEP) is a term used in mechanical ventilation to denote the amount of pressure above atmospheric pressure present in the airway at the end of the expiratory cycle. PEEP is set on the ventilator.PEEP improves gas exchange by preventing alveolar collapse, recruiting more lung units, increasing functional residual capacity, and redistributing fluid in the alveoli. It is used in compromised ventilating patients and is contraindicated in Increased ICT, pneumothorax
