A 65 year old physician with no history of psychiatric problems reports that over the past 3 months he has having difficulty sleeping through the night and has lost his appetite for food. medical eval.

i think the significant factors here are loss of sleep, appetite and loss of significant amount of weight.

"The most appropriate next step in the management of this depressed pt, who has no previous history of psychiatric illness, is a medical evaluation. This pt has symptoms of depression including sleep problems, inappropriate guilt, suicidal ideation, and significant weight loss. Because of this age and because pancreatic caner and other GI cancers not uncommonly present with depression, this pt should be evaluated for such conditions prior to treating this depression. Psychotherapy, antidepressants, and benzo can treat the associated symptoms but do not address this pt's underlying physical problem."
