Classific of Pneum

1. Unilat

2. Bilat

3. HIV

4. Associat with other Ds

5. Allergic

1 Unilat:

a. St Pneum---> MC

b. Smoker--> Klebs

c. Alcoholic--> Hemoph Inf

c. IV Drug--> Staph aur

d. Coccidomycosis--> Unilat infilt + Hilar LNs on same side +/- Erythema Multiform or Nodosum

2. Bilat:

a. Mycoplasma Pn:

Good Pt, ERYTHEMA MULTIFORME MINOR, gm st PMN & no org, Urine serology +ve

b. Legionella:

Pn + GIt ( Abd pain, diarrh), Urine serology +ve

c. Q Fever:

Farmers & Vets, Serology.

3 HIV: Acc to CD4

a. > 200:

i. Regular infec (as before)

ii. TB (Hilar LNs + Night sweats + +ve X-Ray)

b. < 200--- >100:

i. Pneumocystis Carinii

Pneum + Normal C X-Ray...(TMP/SMZ, all--> Pentamid or Atoquav, Prophyl--> TMP/SMZ or Dapsone)

ii. Histoplasmosis:

Miliary affection, PALATAL ULCERS & HEPATO-SPLENO megaly ( + Geogr)

c. < 50:

i. CMV--> Interst bilat affection

ii. Mycobact Avium---> High Fever + Eryhtema Mult minor

4. Associated with Ds:

a. Cystic Fib--> Pseud--> Ceftriaxone + Gentamycin

b. Chidak Higashi---> Staph auer, Pseudom, St Pneu

c. Chronic Glucocorticoid TTT--> Nocardia, TB

d. COPD---> Hemoph Infl, Morax, St Pneum

5. Allergic:
