A 23 year old medical student comes to the emergency room with increased heart rate, sweating and shortness of breath. The student is convinced that she is having an asthma attack and that she will suffocate. The symptoms started suddenly during a car ride to school. The student has had episodes such as this on at least three previous occasions over the past 2 weeks. She has no history of asthma and, other than increased pulse rate, physical findings are unremarkable. Of the following, the most effective longterm treatment for this pt is

An antidepressant
A support group
A benzodiazepine
Buspirone
A beta-blocker

"THis pt is showing evidence of panic disorder. Panic disorder is characterized by panic attacks, which include increased heart rate, dizziness, sweating, shortness of breath, and fainting; and the conviction that one is about to die. Attacks commonly occur twice weekly, last about 30 min, and are most common in young women, such as this pt. While the most immediate treatment for this pt is a benzo bc it works quickly, the most effect long-term (maintenance) treatment is an antidepressant, particularly a SSRI such as Prxil."
