A 70-year-old pt whose wife died 8 months ago reports that he sometimes wakes up an hour earlier than usual and often cries when he thinks about this wife. He also tells you that on one occasion he briefly followed a woman down the street who resembled his late wife. according to BRS beh sci book,page 24

this is normal grief reaction[bereavement]
where there can be
-minor wt loss[1-3 pounds]
-minor sleep disturbances
-some guilty feelings
-Illusions[thimking that seeing deceased person briefly]
-attempts to return to work & social activities
-cries and expresses sadness
-severe symptoms resolve within 2 mths
-moderate symptoms subside within 1 yr
-treatment includes increased calls and visits to the physician,"supportive psycho therapy"confused,and short acting benzodiazepines for temparary sleep problems.

This case-

-died 8 months ago[less than 1yr]
-wakes up an hour earlier than usual and often cries when he thinks about this wife.["minor symptoms"
-briefly followed a woman down the street who resembled his late wife[illusion]
-he has rejoined his bowling team and "enjoys" visits with his grandchildren.[attempts to return to work & social activities]

About psychotherapy,I guess this pt has good family supportnod

Abnormal grief reaction[depression]-is when,

-significant
wt loss[>8pounds]
sleep disturbances
-intense feelings of guilt and worthlessness
-hallucinations and delusions[hearing the dead person talk]
-resumes few,if any work or social activities
-considers or attempts suicide
-severe symptoms persist for >2mths
-moderate symptoms persist for >1yr
-treatment includes antidepressants,antipsychotics,or ECT.
