rupture or torsion ? I am not sure, but will go with ruptured ovarian cyst

I think it is Endometriosis. Her last menses is 3 days ago, that's mean she is probably bleeding and also the endometriosis that could be present on right ovarie or fallopian tube. USG didn't find any mass, just free fluid.

D For torsion, the mass would still be palpated, not tenderness For endometriosis - no sufficient data, and no gobble stone appearance of the cul-de-sac on pelvic exam... In addition, pain would resolve with the beginning of the period.

Yes. I agree that the mass should be there if it is the torsion. The torsion also can't explain the free fluid

D , previously palpated TO mass, now c/o severe pain, and free fluid in the POD, sounds like a rupture.
